The cost for individual health insurance is so expensive that many people cannot afford to purchase it.  The rat (who in reality does not need health insurance) gets a job as a lab rat so he can get medical treatment when sick without having to pay for it.  Rats are used by pharmaceutical companies to test the effectiveness of drugs being developed.
